Life Foundation Home Care’s care model puts the patient at the center of the care service. With their comfort and care being the main focus, we apply our rigorous process to match them with the right caregivers to meet their specific needs. We perform initial and ongoing assessment to ensure their changing needs are addressed and prepare a careplan that is unique to their needs. At Life Foundation Home Care, we believe in open communication and know that we are part of a larger continuum of care service, as such we are always communicating with the family, clinical staff and other stakeholders that are involved in the care of the patient. This is what you should expect if you or your loved one is under our care:
- Initial in-person assessment to go over the needs of the patient including assessing the surroundings and support system
- Custom careplan with input from the patient and/or their designated relative or friend
- Caregiver matching based upon the care needs, including primary and secondary caregivers as needed
- Initial bonding period in the first couple of weeks to make the patient comfortable with their caregivers
- Daily (or as needed) reporting on the services provided by the caregiver and any new observations made by them in patient’s health and routine
- Regular supervisory visits by experienced staff to ensure quality of care standards are met
- Ongoing review of the patient needs to ensure patient needs are being met
