An important note about our role

Life Foundation Home Care provides non-medical personal care and companion services alongside hospice. We are not a hospice provider. Your loved one’s hospice medical team handles clinical care — we provide the personal and emotional support that surrounds it.

Giving family members the opportunity to rest, step away, and attend to their own needs while knowing their loved one is cared for.

Overnight Care

Overnight support so family caregivers can sleep and rest, with a trusted caregiver present through the night.

Family Communication

Keeping family members informed and supported, and providing a consistent, reliable presence in the home.

Meaningful Activities

Engaging your loved one in gentle, meaningful activities — music, reading, reminiscing — that bring joy and comfort.
