About the Program
What is the GUIDE program?
The Guiding an Improved Dementia Experience (GUIDE) program is an eight-year nationwide initiative by the Centers for Medicare & Medicaid Services (CMS) Innovation Center, launched in July 2024. It was created to transform how dementia care is delivered across America — making it more coordinated, more supportive, and more accessible for families.
For the first time, Medicare now covers a comprehensive package of dementia care support services. If your loved one qualifies, these services come at no cost to you under your existing Medicare coverage.

The GUIDE program recognizes that caring for someone with dementia affects the whole family. Here is what eligible participants receive at no cost.
Care Navigation
A dedicated care navigator helps your family find and access local services — meals, transportation, community resources, and more — so you are never figuring it out alone.
Personalized Care Plan
A comprehensive care plan built around your loved one's specific dementia diagnosis, current stage, and individual daily needs.
24/7 Clinical Support
Access to a clinical team around the clock — even at 3am when you are unsure whether to go to the emergency room. Real answers, whenever you need them.
Respite Care
Up to 76 hours of in-home respite care per year covered under Medicare — so the primary family caregiver can rest, recover, and keep giving their best.
Caregiver Education
Practical training and resources on dementia progression, communication strategies, home safety, and how to take care of yourself while caring for others.
Home Safety Assessment
An in-home visit to identify risks and put measures in place that help your loved one remain safely at home for as long as possible.
Eligibility
Does your loved one qualify?
The GUIDE program is available to individuals who meet the following criteria. Not sure whether your loved one qualifies? Call us and we will help you find out.
- Has a confirmed dementia diagnosis (such as Alzheimer's disease) from a clinician
- Is enrolled in Medicare Parts A and B, with Medicare as the primary payer
- Is not enrolled in a Medicare Advantage plan or PACE program
- Has not elected the Medicare hospice benefit
- Is not a permanent long-term resident of a nursing facility
Eligibility is determined by Medicare. Participation is voluntary.
